DESCRIPTION:Health and Wellness Agreements are due annually on September 15th. It is the expectation that all chapters reach 100% completion of these agreements by this deadline. Chapters that meet the expectation will receive full CAP points for this requirement. Chapters that do not meet the expectation may face suspension of their charter and loss of CAP points. \nPlease use the following instructions to ensure timely completion: \nSend the Health and Wellness Agreements form to all active chapter members. Remind members that they must submit the form using their legal first and last name entered within MyDPhiE.\nNew members will complete this form via their New Member Orientation module.\nProvide weekly reminders to chapter members\, including direct outreach as needed\, until all agreements are signed.\nAs Chapter Leaders\, it is your responsibility to ensure all members sign the Health and Wellness Agreements by September 15th. \nFor any questions\, please visit our Knowledge Center and submit a Contact Support Form. To access and submit the Fall Chapter Information Form\, please click the link below. If you have any challenges accessing the form\, please feel free to connect with us via our Knowledge Center Contact Support. Fall 2025 Chapter Information Form Any Leadership Team member may complete the Chapter Information Form which can be submitted and updated as needed. Even if chapters do not have all dates for fall\, please fill it out to the best of your ability by July 15th\, 2025. \nWe encourage Leadership Team members to communicate with each other and connect with the chapter’s Chapter Advisory Board members\, based on the information asked within the form. These forms are used for billing\, programming and more so we ask that you complete them on time. Chapter Information Forms for the Fall term are due no later than July 15th\, 2025. \nAs a reminder\, members with inactive status from the 2025 Spring term returned to active status as of June 1.
